Although MMR and child mortality have declined sharply in Viet Nam, it is estimated that annually there are 580 to 600 maternal deaths and over 10,000 neonatal deaths in Viet Nam. In addition, the inequities and the resulting disparities in maternal health outcomes continue to persist and even to be widening particularly among the ethnic minorities in the hard to reach areas. Maternal mortality rates in the mountainous rural areas are much higher than in the urban areas; and ethnic minority pregnant women are less likely to receive 4 times antenatal care visits, skilled birth attendants (SBA) and post-natal visits than the Kinh majority.
There are several factors contributing to high maternal mortality and low utilization of maternal care services including the SBA amongst the ethnic minority pregnant women. The main causes, however, are the shortage of female SBA who can speak and communicate effectively with the ethnic minority population in their languages; the geographic difficulties and indirect costs of accessing to health facilities; and the cultural perception and traditional beliefs resulting in a common practice of home delivery without SBA amongst the ethnic minority women. To address these issues, the Ministry of Health (MOH) has introduced Ethnic Minority Village Midwives (EMVM) programme in the rural, mountainous and central highland areas with high prevalence of ethnic minority population for more than 20 years. This initiative has been done mainly through selection, provision of intensive trainings with a focus on childbirth practices, and deployment of local women to become EVMVs.
Upon the completion of the programme, it is required to evaluate the relevance, effectiveness, scalability and sustainability of the EMVM initiative which has been implemented by MOH in the mountainous and centrals highland regions of Viet Nam.
